Wellstar Health System is proud to partner with our community by offering meaningful volunteer opportunities. Our volunteers support patients, visitors, and staff at entrance and information desks, gift shops, and a variety of specialty departments.

I understand my completed application, current immunization records and high school counselor recommendation form must be received by the stated deadline. Required vaccines include 2 MMR, 2 Varicella, and 1 Tdap. During flu season (Sept-March) a current flu vaccine is also required. I understand that my services are voluntarily donated to the hospital, and I do not expect to receive compensation of any nature, to include financial compensation, academic credit and/or future employment. By submitting this form, I attest that the information I have provided on this form is true and accurate. As a VolunTeen, I understand that I am required to: Maintain a 3.0 GPA Follow hospital rules and regulations as discussed in orientation Complete TB (tuberculosis) testing prior to my start date Attend a mandatory information session Volunteer for the required length of time Wear designated VolunTeen uniform Must be available to start the program the first week of June.
